Our client who is in the PHARMACEUTICAL INDUSTRY is looking for a CHRONIC CARE TEAM LEADER to join their team.
The company is based in Nairobi.
Qualifications:
- Degree in Science(BSc)/pharmaceutical sciences
- 2-3 years as a SUPERVISOR of a CHRONIC CARE TEAM
- Demonstrated understanding of chronic disease frameworks and the use of best practice guidelines around prevention, risk factor modification and chronic disease management.
RESPONSIBILITIES:
- Providing leadership and direction to the multidisciplinary staff in day to day operations of the team.
- Support the team to deliver high quality, evidence based services to the community.
- Support the implementation of innovative, appropriate, effective and efficient models of care.
